Information we collect
Information you provide directly. When you submit a contact form, request a consultation, send us an email, or call our office, you may provide personal information such as your name, email address, telephone number, and a brief description of what you are seeking help with.
Clinical information. If you become a client, we collect Protected Health Information necessary to provide care — diagnostic, treatment, and billing records. The use and disclosure of this information is governed by HIPAA and the California Confidentiality of Medical Information Act (CMIA), and is detailed in our Notice of Privacy Practices.
Information collected automatically. When you visit our website, our hosting provider (Netlify) and analytics service (Google Analytics 4) automatically collect limited technical information such as anonymized IP address, browser type, device type, city-level location, referring URL, and pages visited. This information is used in aggregate to understand site traffic and improve the site.
Google Analytics configuration. Our Google Analytics property is configured for healthcare use. Google Signals, personalized advertising features, and cross-property audience sharing are disabled across all regions. IP addresses are anonymized at collection, data retention is set to the shortest available period, and no user-identifying information is sent to Google. Pinnacle has executed a Business Associate Agreement (BAA) with Google covering this analytics configuration. If you would prefer that your visit not be measured at all, you can install the official Google Analytics Opt-out Browser Add-on or use a browser-level "Do Not Track" or content-blocking setting.
Cookies. Our website uses cookies and similar technologies only for traffic analysis and basic site functionality. We do not use cookies for advertising, retargeting, or cross-site tracking. You can control cookies through your browser settings; disabling them will not affect your ability to use the site.

We use the information we collect to:

Protected health information (HIPAA)
Pinnacle is a covered entity under the Health Insurance Portability and Accountability Act of 1996 (HIPAA). The use and disclosure of Protected Health Information — including any clinical or treatment-related information — is governed by our separate Notice of Privacy Practices, which is provided to every client at intake and is available upon request.
In summary, we use and disclose Protected Health Information only for treatment, payment, healthcare operations, and other purposes permitted or required by law. Any other use or disclosure requires your written authorization, which you may revoke at any time.

Under HIPAA and California law, you have the right to:
- Access and obtain a copy of your records
- Request amendments to your records
- Receive an accounting of disclosures
- Request restrictions on certain uses and disclosures
- Request confidential communications by alternative means or at an alternative address
- Receive a copy of our Notice of Privacy Practices
- File a complaint with our office or with the U.S. Department of Health and Human Services without retaliation

California residents
California residents have additional rights under California law, including the right to know what personal information we collect about them, the right to request deletion of certain information (subject to retention requirements under HIPAA and other applicable laws), the right to non-discrimination for exercising these rights, and the right to opt out of any sale of personal information. We do not sell personal information.

We maintain reasonable administrative, technical, and physical safeguards to protect the information in our possession. Despite these efforts, no method of transmission or storage is completely secure. Please do not send confidential clinical information via unencrypted email or web form.

Children's privacy
This website is not directed at children under the age of 13, and we do not knowingly collect personal information from children under 13 through this website. Clinical services for minors are provided only with appropriate parental or legal guardian consent in accordance with California law.
